How to use the CMS NDC HCPCS crosswalk to bill J-code drugs
The official NDC HCPCS crosswalk is the quarterly CMS file. Find the NDCs for a J-code or Q-code, read billable units and report the NDC on claims.
How to use the NDC HCPCS crosswalk for J-code and Q-code drugs
The official NDC HCPCS crosswalk is CMS's quarterly file on its ASP pricing files page. Search a J-code or Q-code to find its listed NDCs, package sizes and billable units. As of October 23, 2026, use the October 2026 crosswalk for services from October 1 through December 31, 2026.
Where to get the crosswalk
Manufacturers submit average sales price data at the 11-digit NDC level, while providers mostly bill Medicare for drugs with HCPCS codes (October 2026 file). The file links each NDC to a HCPCS code, and it is not a complete list of billable NDCs (CMS fact sheet).
Files through 2025 are titled "ASP NDC-HCPCS Crosswalk". On the same CMS page, 2026 files are titled "NDC-HCPCS Crosswalk". Mappings change every quarter, so use the quarter that covers the date of service.
For payment limits, use only the NDC-HCPCS crosswalk on that page. Its ASP sheet has 7,403 NDC rows across 877 HCPCS codes. The October 2026 zip also includes OPPS, AWP vaccine and PrEP crosswalks, and those sheets are not interchangeable. Hospital outpatient rates are in OPPS Addendum A and B, and DME suppliers use a separate DME NDC-HCPCS crosswalk (CMS fact sheet). Questions about the crosswalk go to sec303aspdata@cms.hhs.gov (October 2026 file).
How to read a row
Each row lists the HCPCS code, labeler, NDC, drug name, dosage, package size and package quantity. BILLUNITS is the HCPCS units in one item. BILLUNITSPKG multiplies that by the items in the NDC (October 2026 file).
Take J1100 (dexamethasone sodium phosphate, 1 mg). Fresenius Kabi NDC 63323-0165-05 is 25 vials of 5 mL. Each vial is 20 billable units, so the pack is 500. Hikma's J3420 vial 00143-9619-01 (vitamin B-12, up to 1000 mcg, 30 mL) is 30 billable units (October 2026 file).
NDC codes for J3301, J1100, J1885 and other common codes
Descriptors are from the October 2026 HCPCS file. Row counts are from the October 2026 ASP crosswalk. J7620 supplier claims use the DME crosswalk (CMS fact sheet).
| HCPCS | Descriptor | NDC rows, Oct 2026 |
|---|---|---|
| J3301 | Injection, triamcinolone acetonide, not otherwise specified, 10 mg | 41 |
| J1100 | Injection, dexamethasone sodium phosphate, 1 mg | 54 |
| J1885 | Injection, ketorolac tromethamine, per 15 mg | 71 |
| J0696 | Injection, ceftriaxone sodium, per 250 mg | 61 |
| J3420 | Injection, vitamin B-12 cyanocobalamin, up to 1000 mcg | 39 |
| J1071 | Injection, testosterone cypionate, 1 mg | 32 |
| J7620 | Albuterol, up to 2.5 mg and ipratropium bromide, up to 0.5 mg, FDA-approved final product, non-compounded, administered through DME | 27 |
| Q9967 | Low osmolar contrast material, 300-399 mg/ml iodine concentration, per ml | 95 |
| J1050 | Injection, medroxyprogesterone acetate, 1 mg | not listed |
| J7307 | Etonogestrel (contraceptive) implant system, including implant and supplies | not listed |
| J3490 / J3590 | Unclassified drugs / unclassified biologics | no fixed NDC |
A missing row is not "NDC not covered"
J1050 and J7307 aren't in the October 2026 ASP file. CMS says the crosswalk "does not signify coverage or non-coverage of a product", and a MAC may still process the Part B claim after determining the payment limit (CMS fact sheet, citing Pub. 100-04, Ch. 17, §20.1.2). CMS generally lists NDCs with sales in the sales quarter. Other gaps are missing ASP data, an NCD or the Self-Administered Drug List, and CMS removes products that are discontinued, recalled, no longer payable under Part B, or inactive after six quarters (CMS fact sheet). CMS doesn't say why these two codes are absent. For a Medicare claim, ask your MAC for the payment limit.
How to report the NDC on a claim
In medical billing, the NDC identifies the drug product you gave and the HCPCS code is what you bill (October 2026 file). Report the NDC on that line when the payer requires it. Check the current guide first.
On the CMS-1500, use the shaded top of item 24. The NDC qualifier is N4, then the 11-digit NDC with no hyphens (N499999999999), then the NDC unit of measure and quantity (Pub. 100-04, Ch. 26). The HCPCS code goes in 24D. The NUCC 1500 manual puts one space between the NDC and the unit qualifier: UN (unit), ML (milliliter), GR (gram), F2 (international unit) or ME (milligram). CMS lists UN, F2, GR and ML only. Oregon puts a space after N4 (Oregon Health Authority). Confirm both with the payer.
UB-04 form locator 43 takes N4, the 11-digit NDC, then the unit qualifier and quantity with no space (CMS Transmittal 1496). On 837P and 837I, the NDC is loop 2410 LIN03 and the unit of measure is the CTP segment of that loop (Oregon Health Authority). Your companion guide has the exact elements.
HCPCS units and NDC quantity measure different things. In Oregon's example, 4 mg of IV Zofran from NDC 00173-0442-02 (2 mg/mL) is J2405 x 4, with NDC quantity ML2. Oregon uses GR for creams, ointments, and powders, ML for prefilled syringes and unit-dose liquids, and UN for reconstituted vials, tablets and capsules. Two package sizes mean two lines (Oregon Health Authority).
Converting a 10-digit NDC to 11 digits
FDA NDCs are 10 digits: 4-4-2, 5-3-2 or 5-4-1 (21 CFR 207.33). Claims use 5-4-2. Add a leading zero to the short segment, then drop the hyphens (Oregon Health Authority):
| Package shows | Format | Bill as |
|---|---|---|
| 5678-0123-01 | 4-4-2 | 05678012301 |
| 05678-123-01 | 5-3-2 | 05678012301 |
| 05678-0123-1 | 5-4-1 | 05678012301 |
Use the NDC on the package you administered. The FDA NDC Directory updates daily from labeler listings. Inclusion there does not mean FDA verified the listing.
NDC for J3490 and J3590
Neither code has a fixed NDC. Report the administered drug's NDC when the payer requires one. On a Medicare CMS-1500, put the drug name and dose in item 19. Without that narrative or an attachment, the claim comes back unprocessable (Pub. 100-04, Ch. 26).
Who requires the NDC in medical billing
Medicaid requires NDC numbers on rebate-eligible physician-administered drug claims. The Deficit Reduction Act of 2005 required NDC numbers on those claims by January 1, 2007 (SMDL 06-016). A Medicaid NDC lookup starts in the state guide. Oregon denies claims with no NDC, with exceptions such as immunizations, vitamins and oral nutritional supplements (Oregon Health Authority).
On dual-eligible physician claims, Medicare "will not edit, validate, nor process" the NDC. It only passes the number to Medicaid (CMS Transmittal 1401). DME MAC drug claims are the exception, because HIPAA requires an NDC there (Pub. 100-04, Ch. 17).
The 12-digit NDC is coming
FDA's March 5, 2026 rule moves FDA-assigned NDCs to a 12-digit 6-4-2 format on March 7, 2033 (Federal Register). Through March 6, 2036, FDA does not intend to object to 10-digit labels, but systems should handle 10-digit and 12-digit NDCs. The 11-digit 5-4-2 billing format is not the same as FDA's new 12-digit format, so billing systems should plan for the change (FDA NDC format page).
